We are located on the campus of the University of Arkansas for Medical Sciences in Little Rock, Arkansas. The address is 4301 West Markham Street, north of Interstate 630.
Our office is inside the Outpatient Center on Jack Stephens Center on Jack Stephens Drive, southeast of the main hospital building.
The closest entrance is off I-630. Take the Cedar/Pine Street exit. Get onto Pine Street and follow it to West Capitol. Turn left. The Outpatient Parking Deck is at the corner of West Capitol and Jack Stephens Drive on the right. The Outpatient Center is across the street. Our office is located on the 4th floor.

Initial Appointments
Most patients can expect to have Vascular Ultrasound tests at the time of their initial appointment. These tests are conducted on site and in many instances prior to the clinic appointment.

- A list of all medications you are currently taking, including dosage and frequency. Please list non-prescription medicines and supplements as well.
- The name and phone number of your primary care doctor.
- The name and phone number of the referring doctor (if different from your primary care doctor).
- Any X-ray films or lab results provided by the referring doctor.
- Health insurance card(s) or other information about your health coverage.
